Answer each question honestly. GenoPept can only supply researchers who use products solely for laboratory research.

1Are you purchasing GenoPept products solely for laboratory research use — in-vitro, analytical or other non-clinical research?
2Will any product be used in or on humans — including consumption, injection, or any personal, clinical, diagnostic or cosmetic use?
3Will any product be used for veterinary purposes or administered to animals?
4Will you resell or supply any product to others for human or veterinary use?
5Are you 18 or over and competent to handle, store and dispose of research materials safely?

Declaration

I declare that:

  1. All information I have given in this application is true, accurate and complete.
  2. Every product I purchase from GenoPept is for laboratory research use only (RUO). No product will be used for human or veterinary purposes — including consumption, injection or any other administration to humans or animals, or any diagnostic, therapeutic, clinical or cosmetic use.
  3. I am 18 or over and I am a researcher, or an authorised representative of the institution named in this application, competent to handle research materials and responsible for their safe handling, storage and disposal.
  4. I will not resell, supply or give any product to anyone for human or veterinary use.
  5. I will tell GenoPept if any of this information changes. I understand GenoPept may suspend or withdraw my Approved Researcher status, and refuse or cancel orders, if it has reason to believe this declaration is untrue or has been breached.
  6. I have read and accept the Research Use Only policy and the Terms & Conditions.
